Q & A

If the offeror already holds a contract in a different district, is the offeror required to submit copies of monitoring/compliance/audit/performance reports?

Yes - according to the Statement of Work (Section K 2 a); the offeror is required to provide copies of all monitoring/compliance/audit/performance reports for the previous 24 months from all federal, state and local agencies. Offerors who are currently awarded an agreement with the USPO/USPOS (i.e. US Probation - Eastern District of Michigan) conducting the solicitation are not required to provide copies of the monitoring reports.

Does the offeror have to have a current license in the State of Michigan at the time of submitting the proposal?

Yes - According to Section K 3 (Offeror's Staff Qualifications - Attachment C); the offeror shall verify the information is accurate and that any required licenses are current. Therefore, licenses must be current at the time the proposal is submitted and remain current for the duration of the contract.

What is the quality of services expected per month?

The RFP indicates the Estimated Monthly Quantity. Example: 4 individual units per month = 2 one-hour sessions per month (1 unit = 30 minutes). Each project code/service has an estimated monthly quantity listed.

How many vendors will be awarded a contract in each catchment area?

The RFP indicates 1 to 3 vendors per catchment area. Example: If multiple technically acceptable bids are received, and the department has a need in more than one county (same catchment area); awards could be issued to up to 3 vendors.

We do provide most of the services listed in the RFP, including SUD and MH assessments, SUD counseling, cognitive behavioral groups, case management, family counseling and co occurring disorders counseling. We however do not provide urine drug testing, physical medical services or psychiatric services. We do maintain referral relationships with agencies and medical providers for those services. Given that we do not provide UDS services, medical or psychiatric, would we be eligible to apply to be a vendor for the services we do provide?

Urine drug testing is the collection of urine screens by one of vendor's staff and mailed to our national lab in Louisiana. Our district will supply the testing supplies (i.e. cups, chain of custody forms, labels and mailing envelope). The vendor awarded the contract is not responsible for testing the sample. The vendor is responsible for maintaining a code-a-phone system, observing, collecting, mailing, and reporting the results to the assigned officer. An account would be set up with our lab for result reporting purposes.

Physical Examination Services and Psychiatric Services - can be subcontracted. It is noted, any subcontractor shall be fully licensed/credentialed and maintain compliance with State of Michigan statutes, regulations, and guidelines for providing the scope of services and direct treatment.

An offeror must be capable of providing all services identified in Section B. If the offeror is unable to provide a service identified in Section B and does not identify/subcontract with someone to provide the service, the offeror will be technically unacceptable.

Do I have to submit to all 3 requests services in the proposal?

Each proposal has a designated catchment area. You can submit a bid for the specific catchment area that applies to the location of your business. Other than polygraphs, all services must have a physical location in the catchment area. Example: City of Detroit contracts require a building within the City of Detroit to provide in person services.

Can I submit a proposal for the mental health treatment, substance abuse treatment, substance abuse testing, and sex offender treatment? Or can I submit the mental health treatment portion of the proposal?

Each RFP provides project codes for services. ALL services per RFP must be provided for a proposal to be considered technically acceptable. Offerors cannot "pick and choose" which services they would like to provide.

Can another therapist, licensed counselor, psychologist perform the sex offender treatment portion of the RFP?

All persons providing services must be included in the proposal under Section L.1, Preparation of Staff Qualifications (Attachment C).

The proposal states to complete blocks 10-15 on pg 1 (Solicitation Offer and Acceptance). Do I also complete blocks 9, 16 and 17A?

Section L indicates which blocks must be completed. Block 10 - is only if we request a Modification to the contract. It remains empty. Section L - Blocks 9, 16 and 17A are not noted for the offeror to complete. They remain empty.

If the estimated monthly quantity on pg. 6 for mental health individual counseling is different than what is listed, do I just white that out and put in the different number? Or, can it be different?

The Estimated Monthly Quantity is US Probation and Pretrial Services "best estimate" as to how many units of service would be requested from the vendor in a month. That number cannot be changed by the vendor.

Should Administrative Fee on pg. 8 stay listed as Unknown?

Yes

Should the actual paper for sections K.1 and K.2 on pg. 132 be submitted, or just the information from it in a different format?

Yes, submit sections K.1 and K.2 on the form provided within the RFP.

Is it stating 39 clients for the estimated monthly quality on pg. 6?

39 units. 1 Unit = 30 minutes; 1 hour = 2 units.

Is it stating 1 visit per month per family for mental health family counseling? Also, is family defined as 2 or more clients in one session together?

The Estimated Monthly Quantity for family counseling is 1 unit per month. 1 Unit is 30 minutes. Family sessions are not routine but may be necessary to support the individuals progress in treatment. Under Code 6030 Mental Health Family counseling, family is defined as one or more family members.

What do administrative fees include?

Code 6041 Administrative Fee is 5 percent of the actual funds expended to acquire the medication (i.e. via a pharmacy or other source).

Is pg. 147 necessary to complete if additional staff will be helping, particularly in administration?

Staff who will be providing services for the project codes must be listed on Attachment C - specified by project code for each staff member.

Pursuant to the bid, am I including last State audit only or should I also include recent Fed ones as well.

Offerors who are currently awarded an agreement with US Probation (ED/MI) are not required to provide copies of USPO monitoring reports from (ED/MI). Reports from our agency are not needed.

Does Attachment C require those staff members who are working the front desk/conducting urine screens.

Staff who will be providing services for the project codes must be listed on Attachment C - specified by project code for each staff member. If staff is conducting urine screens, they need to be listed under Codes 1010 and 1011.

Are therapists required to utilize the sex offender forms on Pages 71-97 of the RFP?

Sex Offender Forms Section J.6 (APA Model Sex History Disclosure Polygraph Questionnaire) on Pages 71-97 are utilized in conjunction with the USPO, Person Under Supervision, and Polygrapher in consultation with the therapist for polygraph examinations.

In Section A, Block 10, what are we supposed to put if anything?

Block 10 - is only if we request a Modification to the contract. It remains empty.

In Section A, Block 11, should I put my name or the name of my company or both?

Section A, Block 11 is the name of the company.

In submitting the proposal, should we just remove pages 4-94 (Sections C-J)?

Submission of the signed proposal. Page 136 Proposal Submission. NOTE: The Offeror shall not submit solicitation sections C,D, E, F, G, H, I and J as part of its proposal.

Since Sections L (pages 97-106) and Section M (pages 112-116) appear to be directions, should I remove these from my proposal submission?

Yes you can remove the instructional pages from the proposal. However, the attachments from Section L must be included.

I listed an employee on Attachment C as he works with me. Should I also list him as a subcontractor on Attachment A?

Staff who will be providing services for the project codes must be listed on Attachment C - specified by project code for each staff member.

Are secretaries, grant preparer team members, etc. considered proper to pay under administrative fees?

No. The administrative fees do NOT cover secretary, grant preparer or team member's pay. The administrative fees are 5% of the actual funds expended for the service. Example: Prescription Medication costs $10.00. The administrative fee would be 5% of $10.00 ($0.50).

In light of Question #1, does Section C have to be completed? I am asking is there an exception in that case, since the RFP said NOT to complete Section C, just sign that it would be completed (per pg. 144). Do I just need to have pg. 144 signed, and list the administration to be included and paid?

Section C has nothing to complete. Section C details the expectations for each Project Code. ALL attachments must be signed, dated and submitted with the proposal.

Do I include the observation cost in the price of the drug test collection ? Or is that observed cost indicated in invoices

Per Section C of the RFP: Project Codes 1010 and 1011 include the observation, collection, packaging, storing, mailing, maintaining code a phone, maintain a testing log, report results and non-compliance to the officer.
